Request a duplicate driver’s license
Step 1: Print and complete the required form
Step 2: Make an appointment

Step 3: Get your driver’s license in the mail
You will get a temporary license at your appointment. Your permanent driver’s license card will arrive through the mail.
Your new license will only be mailed to the mailing address on file with the DMV. If you have mail forwarding, you will NOT receive your new driver’s license.
For more on this, please see Central Issuance.
After visiting an office, you can track the delivery of your new card to see when it will arrive.
